The Maricopa County Department of Public Health is partnering with SRPMIC and Salt River Schools to provide FREE flu shots! Our flu vaccine clinic will be held Wednesday, Nov. 9, from 9 a.m. to 12:30 p.m. in the ECEC parking lot (4836 N Center St, Scottsdale, AZ 85256).
No registration is required ahead of time. The vaccine clinic is open to all Salt River Schools staff and students, as well as Community members.
Click here to download and fill out the consent forms for your child. Children must be accompanied by a parent/guardian or bring a signed consent form (accompanied by SRS staff) in order to receive a flu vaccine.
Insurance is not required, but if you have an insurance card, please bring it to the event or put the member ID number on the consent form.
